Here I share about students’ portfolios format for XII grade. Hopefully it is useful for English teachers (Especially for teachers who teach English in Indonesia).

How we use it will be explained below.
1. Give this document to students!. (We may give it in the middle of even semester)
2. Ask students to enclose all documents they have! (e.g Listening, Speaking, Reading, Writing, Grammar etc). it must be put after the main documents.
3. Tell your students to answer all questions themselves in the main documents!
4. Ask students to give their portfolios documents to their parents. And then ask them to give comment about their child’s documents and sign it!
5. Ask them to bind it.
6. Ask them to submit it to teachers.
